Health Requirements

Health Requirements for School Entry 2026-2027

Troy School District has a first-day exclusion policy. Per School Board Policy 7:100- Unless an exemption or extension applies, students may not attend school until the required health forms are presented. Please see the Troy School District website's Health Services section for more information.

1. PHYSICAL EXAMINATION: Required for all students entering grades EC, Pre-School, Kindergarten, 6th Grade, and students who are New to the District students

  • The physical exam form must be submitted to your school nurse by August 1st.  Physicals dated within one year of school entry, and include all Illinois State Health Exam requirements, will be accepted.
  • The Health History portion, at the top of page 1 on the form, must be completed, signed, and dated by the parent/guardian.
  • Incomplete examination forms will not be accepted and will be returned.
  • Keep a copy of all forms for your records.
  • A sports physical may NOT be used as a 6th Grade physical; however, a 6th Grade physical can be used for sports, if the “Interscholastic Sports” box is checked “Yes”.

2. PROOF OF IMMUNIZATION: Required for all students entering grades EC, Pre-School, K, 6th, and New to District students

*If a medical reason prevents a student from receiving a required immunization by the first day of school, the student must present, by August 1st, an immunization schedule and a statement of the medical reasons causing the delay. The schedule and statement of medical reasons must be signed by a physician, advanced practice nurse, physician assistant, or local health department responsible for administering the immunizations.

*If your child has not received the vaccine(s) by the date on the appointment card, they will not be allowed to attend school until proof of the IDPH-required immunizations is presented to the school.            

3. EYE EXAMINATION: Required for Kindergarten and New to District students

  • Due by October 15th of the relevant school year

4. DENTAL EXAMINATION: Required for Kindergarten, 2nd, and 6th Grade students

  • Due by May 15th of the relevant school year

Important Health Forms: The following forms are only valid for one (1) school year.

New forms must be completed and signed by the parent/guardian and the child’s physician at the start of every school year:

  • School Medication Authorization Forms
  • Allergy Action Plans
  • Asthma Action Plans
  • Diabetes Care Plans
  • Authorization to Provide Diabetes Care
  • Seizure Action Plans
  • Health Plans - Individualized to Students
    • If your student requires a Health Plan, please schedule a meeting with the school nurse at least one week before school begins to ensure your child’s safety.
